The Pentagon has reversed course on flu shots for new recruits after an outbreak at the Air Force’s main basic training hub. Boot camps for all the military services are once again requiring the flu vaccination for all recruits after Defense Secretary Pete Hegseth made the shot optional for the military at the end of April.

What happened

The development comes amid a growing, weekslong flu outbreak at the U.S. Air Force’s boot camp at Lackland Air Force Base that has sickened nearly 300 people. As of Wednesday, the number of cumulative cases linked to the outbreak at Lackland, part of Joint Base San Antonio, had reached 275, up from 222 cases reported on Tuesday.

Four people had been hospitalized as of Tuesday. The outbreak follows an April decision by Defense Secretary Pete Hegseth to make the annual vaccine optional for service members.

How vaccination rates collapsed

When the outbreak in Texas first began, the Air Force acknowledged that only about 40% of trainees opted to take the flu vaccine — a total that used to be 100%, because it wasn’t optional until two months ago.

The Air Force now plans to vaccinate every recruit in the current class and all new arrivals. Additionally, the Army is preparing to extend its requirement to troops deploying overseas, first responders, child care workers, healthcare personnel, prison staff and soldiers in certain large training exercises.

Why recruits are a flu hotspot

Recruits are exposed to high stress, low sleep, and close contact conditions for weeks. They typically sleep together in large, open rooms, shower communally, and conduct much of their instruction and inspections in close contact.

U.S. Department of Defense research in 2026 found that recruits are hospitalized for the flu far more often than other troops. The death of a recruit at the base is under investigation, according to an Air Force statement. Keon McDaniel was in his sixth week of Basic Military Training when he experienced a medical emergency on June 12. He was transported to Brooke Army Medical Center, where he subsequently passed away. The cause of the medical emergency is currently under investigation, and a comprehensive medical review is being conducted to determine the facts, according to the Air Force. It’s unclear if his death is linked to the flu outbreak.

The policy reversal

Undersecretary of Personnel and Readiness Anthony Tata has granted exceptions to the flu vaccine policy to the Department of the Army, Department of the Navy, which includes the Marine Corps, Department of the Air Force, which the Space Force falls under, National Security Agency, and Defense Health Agency.
