Trump Announces $90 Checks for 20 Million Seniors to Cover Medicare Premiums
President Trump announced Friday that over 20 million "wonderful seniors" will receive $90 checks this month to help cover Medicare premiums.
The one-time payments go to Medicare Part B enrollees, delivered in early October via direct deposit for those with bank details on file, or as paper checks mailed to the address Medicare has on file. Trump described the amount as "nearly $100" in his Truth Social post; a White House fact sheet confirmed the exact figure is $90. CMS later put the eligible universe at 20.8 million.
Funded from the "slush fund"
The money comes from the Medicare Improvement Fund, created by Congress in 2008 with $2 billion for fee-for-service improvements. Trump called it a "pointless 'Slush Fund'" and said no previous administration ever used it for direct payments to seniors. Prior Congresses took $20.7 billion from it as part of the Affordable Care Act.
Not everyone qualifies. Seniors whose Part B premiums are already paid by Medicaid are excluded, as are higher-income enrollees who pay the Income-Related Monthly Adjustment Amount, about 8% of Part B enrollees. Roughly half of Medicare enrollees in Medicare Advantage plans also appear excluded, per KFF's Larry Levitt.
Context on the dollars: the standard 2026 Medicare Part B monthly premium is $202.90, with an annual deductible of $283. The $90 covers about 44% of one month's premium. CNN noted Part B premiums actually jumped nearly 10%, or $17.90, from 2025 to 2026, the largest increase in four years, contradicting Trump's claim that he reduced premiums by significant amounts.
The total payout runs roughly $1.8 to $2 billion, timed to land before the November 3 midterms.
Separately, Trump said $500 "Working Families Obamacare Refund" checks for roughly one million ACA enrollees across 30 states had gone out, premised on Biden-era overcharged user fees. He tied the announcement to his $5,000 "Trump Dividend," saying it "will be distributed to every US citizen if, and when, the Republicans win the Midterm Elections!"
