Pentagon Probe: AI Overreliance, Stale Intel Led to Iran School Strike

A Pentagon internal investigation has concluded that outdated intelligence, compressed target-review timelines, and an overreliance on AI targeting tools — including Palantir’s Maven Smart System — contributed to the Feb. 28, 2026 U.S. Tomahawk strike that destroyed an elementary school in Minab, Iran, per Bloomberg.

What the investigation found

Within hours of the Feb. 28 missile attack that hit an Iranian school, some Pentagon personnel knew the U.S. was responsible, and the probe found what officials describe as a cascade of preventable failures.

Two Tomahawk missiles struck the Shajarah Tayyebeh Elementary School and its grounds, killing more than 150 people, including at least 123 children. The school was built on land that was part of a military compound in the southern Iranian town of Minab, making it the deadliest American military targeting error of the 21st century in terms of child casualties.

The school had been separated from a former Iranian military site years earlier, but updated information about the location did not reach the authoritative targeting database.

The Palantir Maven angle

Officials said some U.S. Central Command personnel relied too heavily on the Maven Smart System, made by Palantir Technologies. The AI-enabled platform combines more than 150 data inputs to support military planning and command decisions.

Investigators found that personnel overrelied on Palantir’s Maven AI system, which they incorrectly believed would flag stale intelligence, while the target database continued to classify the site as a military facility.

Palantir pushed back, saying that expectation did not reflect the system’s responsibility. A company spokesperson told Bloomberg that Palantir was not responsible for the underlying intelligence or for identifying deficiencies in that data, and said there was no evidence its software caused the Minab strike.

Pressure from the top

Internal pressures contributed to the failure, including an administration demand for over 1,000 strikes within 24 hours and a 90% reduction in civilian-harm-mitigation teams.

On March 12, more than 120 Democrats in the U.S. House wrote to Defense Secretary Pete Hegseth asking about the Minab school strike and the role that artificial intelligence, including the Maven Smart System, played in identifying the site as a target. The Pentagon has not responded publicly to such questions, citing the ongoing investigation.

After the Minab strike, Palantir built new capabilities into Maven to re-review underlying intelligence and flag inconsistencies.
